Psychologist: Good morning! Thank you for coming in today. This is your
first meeting with me, so I will ask you some questions. We call this an intake
interview. It helps me understand your situation better. Let's start with some
general questions.
First, I would like to know a little about your background. Where did you
grow up? Tell me about your family. Is there any family history of mental health
problems? For example, did your parents or grandparents have depression or
anxiety?
Also, I need to ask about your medical history. Do you have any health
problems? Do you take any medicine? Have you visited a doctor recently?
Now, let's talk about your current issue. What problem brought you here
today? Can you describe it?
Client: I have been feeling very sad and tired all the time. I don't enjoy
things I used to love. I think it started about three months ago.
Psychologist: Thank you for sharing. So the onset of the problem was about
three months ago. And what about the frequency? How often do you feel this way?
Every day? A few times a week?
Client: Almost every day. Some days are better, but most days I feel low.
Psychologist: I understand. And the duration? When you feel sad, how long
does it last? A few hours? The whole day?
Client: Usually the whole day. I wake up feeling tired and the feeling stays
with me.
Psychologist: On a scale from 1 to 10, how would you rate the severity of
your problem? One means very mild, ten means extremely severe.
Client: I would say 7 or 8.
Psychologist: That is quite high. Now let's talk about the impact of this
problem on your life. How does it affect your daily functioning? For example, can
you go to college, do homework, meet friends?
Client: It's very hard. I miss classes sometimes because I can't get out of bed.
I stopped doing homework. I don't want to see anyone.
Psychologist: What about your sleep pattern? Do you sleep well?
Client: No, I sleep very badly. I wake up at 4 a.m. and can't fall asleep
again.Psychologist: And your appetite? Have you noticed any changes?
Client: I don't feel hungry. I lost some weight.
Psychologist: What about your social life? Do you spend time with friends?
Client: Not anymore. I avoid them. I don't answer messages.
Psychologist: Let's talk about how you usually cope with difficult feelings.
What are your coping mechanisms? What do you do when you feel sad or
stressed?
Client: I used to draw and listen to music. But now nothing helps. I just stay
in bed and scroll on my phone.
Psychologist: I see. And your support system? Who can you turn to when
you need help? Family? Friends?
Client: My mother, maybe. But I don't want to worry her.
Psychologist: It's important to have people you can talk to. Now, let's think
about the future. I would like us to do some goal setting together. What would you
like to achieve from our sessions?
Client: I want to feel better. I want to enjoy life again.
Psychologist: That is a very good goal. Based on everything you told me
today, I suggest we create a treatment plan. We will meet once a week. In our
sessions, we will work on your coping skills, talk about your feelings, and find
small steps to improve your daily routine. Does that sound good?
Client: Yes. Thank you.
